You may need to login before filling up X-Form then follow the steps provided below:
- Select contact reason 'Wallet withdrawal'
- Select reason 'I can not activate Lazada Wallet'
- Select refund method (Bank Transfer or Store credit/Voucher)
- Fill up all required information and click submit once done.
NOTE: Please attach your passport and bank account detail on the form if prompted by the system, we'll ensure that all of your personal detail will be safe and on privacy.
After submission, our relevant team will update you regarding the request via email within 3-5 business days.
