вот якщо по першій ссилці робити, то після вставляння коду у фанкшинс, форма пропадає взагалі)
Проверяйте, может вы еще какие-то правки перед этим вносили, что-то меняли в шаблоне вывода формы. Проверил только что на рабочем проекте - код нормально отрабатывает (шаблон Rehub). Возможно в вашем шаблоне кастомизирован хитро вывод и что-то конфликтует.

в шаблоні кастомізовано, + в самому файлі прописані аргументи, я робив через дочірню тему і усі зміни там не проводились, а потім вдалося внести свої аргументи в файлі основної теми і зміни внеслись, проблема вирішена, дякую)
